The Report of Birth is a declaration of the person furnishing information on the birth abroad of a Filipino child. It is important for Filipinos permanently residing abroad to promptly register with the Consulate the birth of a child in order for such a birth to be registered with the Office of the Civil Registrar General in Manila.  When a child is born abroad to parents who are both Filipino citizens or to one (1) parent who is a Filipino citizen, i.e. one who has not been naturalized as a citizen of a foreign country, the child’s birth must be immediately reported.

The birth must be reported to the Embassy within twelve (12) months after its occurrence. When the parents neglect to report the birth within twelve (12) months, the birth may nevertheless be recorded upon the determination of the consular officer of satisfactory evidence on the authenticity of the report. In these cases, the person who executed the report shall furnish the Embassy with an explanation surrounding the delay in reporting the birth, including an Affidavit of Delayed Registration of Birth.

ONLY births that happened in the following places can be registered or reported at the Philippine Embassy in Abuja: Nigeria, Benin, Burkina Faso, Cameroon, Central African Republic (CAR), Côte d’Ivoire, Equatorial Guinea, Gabon, The Gambia, Ghana, Liberia, Sierra Leone, and Togo.


REQUIREMENTS:

Present original documents and submit four (4) photocopies of each. Kindly collate and arrange into sets of four (4) according to the order of documents below:

  1. Report of Birth Form (ROB Form Rev. 01 24 April 2018)
    • All four (4) copies of the form should be printed in A4 paper, originally filled out.
    • Entries must be TYPEWRITTEN OR PRINTED LEGIBLY.
    • Entries must be at the time of birth of the child/subject.
  2. Birth Certificate issued by the country of birth (original copy should be submitted to the Embassy)     
    • For birth certificates not in English language, kindly submit an official English translation
  3. First passport or Travel Document of the Child/Subject
    1. Photocopies of bio data pages
    2. In case of unavailability, execute an Affidavit of Non-submission of Document and attach a copy valid/recent passport or any valid ID.
  4. Authenticated Marriage Certificate of Parents from the Philippine Statistics Authority (PSA) / Foreign Marriage Contract

* For unmarried parents:

a) If child/subject is using the surname of the father, kindly submit an Affidavit to Use the Surname of the Father (AUSF) to be executed by the mother and Affidavit of Acknowledgement of Paternity to be executed by the father. If the two (2) affidavits will be executed in the Philippines, these should be registered first as Legal Instruments at the Local Civil Registry Office of the place of execution and authenticated by the DFA. If executed abroad, these should be registered first at the Foreign Service Post (FSP) of the country of residence of the parent(s) or at the FSP nearest the place of residence.

b) If child/subject is using the surname of the mother, kindly submit the Birth Certificate of

the mother.

  1. Passport of Parents (valid at the time of the birth of the child) + current/valid passports of both parents
    1. Photocopies of bio data pages
    2. In case of unavailability, execute an Affidavit of Non-submission of Document and attach a copy valid/recent passport or any valid ID [(i.e. Green card or permanent resident card, copy of visa (at the time of birth of the child), working permit if working abroad at the time of birth of the child]
    3. For applicants born in USA and Canada, kindly submit any of the following documents of parent/s:
      • USA/Canada Visa
      • USA/Canada Working Permit
      • Green Card of Permanent Resident Card
    4. If Filipino parent/s had acquired foreign citizenship (i.e. American, Australian, British or Canadian) and re-acquired Philippine citizenship, submit certificate of naturalization of parents and re-acquisition certificates of parents and of the child. If the child is 18 years old and above at the time of parent’s re-acquisition of Philippine citizenship submit an Identification Certificate for Filipino citizen issued by the Bureau of Immigration.
  1. Four (4) recent passport size photos of the child/subject
  2. Notarized Affidavit of Delayed Registration of Birth [required only for children/subjects who are more than one (1) year old]
  3. Notarized Affidavit of Two (2) Disinterested Persons [required only for children/subjects who are more than one (1) year old]
  4. Negative Certification of Birth Record from the PSA (CRS Form No. 1)

PROCEDURE:

Send by mail or submit in person to the Embassy the following documents:

1. Four (4) original duly accomplished Report of Birth Forms (either type written or legibly handwritten) by any of the following:

  1. attending physician;
  2. attending nurse or midwife;
  3. Filipino parent or Filipino parents;
  4. the person whose birth is being reported if over 18 years of age; or
  5. the foreign parent only if none of the above can accomplish the report.

If sending by mail outside Nigeria, all four (4) Report of Birth Forms must be completed before a notary public, who in turn will notarize the Report of Birth forms.

2. Original and four (4) photocopies of the child’s Certificate of Birth issued by local health authorities or by the Local County Registrar

3. Original and four (4) photocopies of the data page of the Philippine passport(s) of Filipino parent(s).

4. In case the Filipino parent became a naturalized citizen of another country after the time of child’s birth but before reporting it to the Embassy, present also Original and four (4) photocopies of the naturalization certificate and proof of Philippine citizenship (example: Philippine passport or Birth Certificate issued by the Philippine Statistics Authority.

5. Original and four (4) photocopies of the data page of the foreign passport or foreign driver’s license of foreign parent.

6. Original and four (4) photocopies of the father’s birth certificate. If the father is Filipino, the birth certificate must be issued by the Philippine Statistics Authority (PSA, formerly NSO).

7. Original and four (4) photocopies of the birth certificate of the mother. If the mother is Filipino, the birth certificate must be issued by the Philippine Statistics Authority (PSA, formerly NSO).

8. Original and four (4) photocopies of the marriage certificate of parents, if applicable. If the parents were married in the Philippines or had their foreign marriage reported with a Philippine Consulate over a year from reporting child’s birth, the marriage certificate must be issued by the Philippine Statistics Authority (PSA, formerly NSO

9. Processing fee of USD25.00 or NGN11,250.00 (non-refundable), payable in cash or bank transfer to the Philippine Embassy. Personal check and credit cards are not accepted.


NOTE:

Additional documents may be required by the action officer from the applicant upon evaluation of the requirements to determine the citizenship, identity or eligibility of the child for registration of birth under the Philippine law.
