ONLY deaths that happened in the following places can be registered or reported at the Philippine Embassy in Abuja: Nigeria, Benin, Burkina Faso, Cameroon, Central African Republic (CAR), Côte d’Ivoire, Equatorial Guinea, Gabon, The Gambia, Ghana, Liberia, Sierra Leone, and Togo.
REQUIREMENTS:
Present original documents and submit four (4) photocopies of each. Kindly collate and arrange into sets of four (4) according to the order of documents below:
- Report of Death Form (ROD Form Rev. 01 24 April 2018)
- All four (4) copies of the form should be printed in A4 paper, originally filled out.
- Entries must be TYPEWRITTEN OR PRINTED LEGIBLY

Death Certificate (original copy should be submitted to the Embassy)
For death certificates not in English language, kindly submit an official English translation
- Certificate of Sealing of Casket
- Autopsy/Embalming Report
- Photocopies of Passport of the deceased (data pages only)
- Affidavit of Delayed Registration of Death
- Report of Death Form (ROD Form Rev. 01 24 April 2018)
PROCEDURE:
Send by mail or submit in person to the Embassy the following documents:
1. Four (4) original duly accomplished Report of Death Form (ROD Form Rev. 01 24 April 2018) (either type written or legibly handwritten) should be executed by next of kin or the attending physician or nurse or their duly designated representative. The Report of Death form must be notarized with the notary public signing AND acknowledging that the one reporting the death personally appeared before the notary public.

2. The original copy of the Certificate of Death issued by the local health authorities must be presented. The Certificate of Death should also be reproduced in four copies.
3. Valid Philippine passport of the deceased must be submitted, also reproduced in quadruplicate (four copies), in order to determine Filipino citizenship. In the absence of the valid passport, any other form of identification showing Filipino citizenship such as:
a) Dual Citizenship (RA 9225) Documents,
b) NSO/PSA Birth Certificate with NOTARIZED affidavit of next of kin certifying that the deceased has not acquired foreign citizenship, or
c) expired Philippine passport with NOTARIZED affidavit of next of kin certifying that the deceased has not acquired foreign citizenship.
4. Processing fee of USD25.00 or NGN11,250.00 (non-refundable), payable in cash or bank transfer to the Philippine Embassy. Personal check and credit cards are not accepted.
NOTES:
Additional documents may be required by the action officer from the applicant upon evaluation of the requirements to determine the citizenship, identity or eligibility of the child for registration of death under the Philippine law.
